Menu Please list your Festival Menu and prices for review and list your equipment on this application form. Restaurants will be chosen by the order we receive your application and at the discretion of our organizers. We are looking for diversity of food to complement a variety of beer and the opportunity to support our local restaurants. You will have restaurant type exclusivity. Registration There is a limit of only SIX (6) restaurant booths in participation. Apply now to ensure your spot at Gulf Brew 2008. Please list on application all employees of the restaurant who will work at the festival. Each person listed will receive a worker's badge at the check in. Please have staff bring photo ID to the festival. They will be asked for a photo id prior to receiving a badge. No one under 18 is allowed to enter the festival and no one under 21 is allowed to consume alcohol. Help us feed our hard working volunteers. If you are willing to offer something off your menu to us for $2 or less, please list below the menu item and price. We will supply the food voucher & upon receipt of the food vouchers from you (after the festival) we will remit a check to you.

Sales | 1. All food sales are on a CASH ONLY basis. | | 2. Vendor is limited to the sales of up to FOUR items, and only preauthorized items may be sold. NO EXCEPTIONS. | | 3. Vendors will serve sample-size portions with the recommended price not to exceed $10 and at least one item on the menu priced at $3 or below. |
Insurance Vendor shall obtain commercial, general, or comprehensive liability insurance including product liability insurance applicable during vendor’s operations at Gulf Brew, insuring those operations with limits of not less than one million ($1,000,000.00) per occurrence and naming Gulf Brew and the Acadiana Arts Council as an additional insured. Vendor shall furnish Gulf Brew with evidence of such insurance no later than June 1, 2008. If proof of liability insurance is not provided by this date, the agreement will be forfeited and reassigned. Gulf Brew shall have no obligation whatsoever to insure, provide, or obtain insurance for food vendor. In any case, vendor will not be permitted to enter the Festival grounds to set up or occupy booth space without having the required insurance. ATTENDING THE FESTIVAL
Before the Event

During the Event
| 1. Event begins at 3 p.m. and will conclude at 10 p.m. | | 2. Doors will open at 3 p.m. to ease the rush on the entry tent and for VIP ticket holders. | | 3. Food will be available for purchase beginning at 3pm. | | 4. Every guest will be required to show ID at the entry tent. No one under the age of 18 is allowed to enter the festival grounds and no one under 21 will be allowed to consume alcohol. | | 5. Live music will be performed throughout the event. | | 6. Additional draft beer and food will be sold during the event. |
After the Event
| 1. Break down begins at 9:30 p.m. In order to receive your deposit back, please leave your booth in the condition in which it was found! | | 2. 10 p.m. Gulf Brew is officially over…begin making plans for next year and THANK YOU! |
